Our school is participating in the Vermont Youth Risk Behavior Survey (YRBS) sponsored by the Vermont Department of Education and the Vermont Department of Health. The survey gathers information about health risk behaviors of 6th through 12th grade students. The survey includes questions about nutrition, alcohol, tobacco and other drug use, physical exercise, injuries, and sexual behavior. Schools, communities and state agencies will use the results of the survey to evaluate programs, plan new programs and to secure future funding.

We are planning to administer the survey on Tuesday, April 11, 2023.
The federal Centers for Disease Control and Prevention, in conjunction with state and local school officials, developed the YRBS. It also has the support of many national organizations including the National PTA.
The YRBS is anonymous, and survey procedures have been designed to protect your child’s privacy. The names of students who take the survey and their survey responses will not be disclosed to any school employee nor will any student's name or survey responses be divulged in a report of the results. We would like all students to take part in the survey; however, the survey is voluntary. No student is required to take the survey.
The Southwest Vermont Supervisory Union has adopted a policy in compliance with the No Child Left Behind Act in regards to student surveys. You have the right to inspect certain surveys as well as the right to opt your child out of participation in certain surveys, including the YRBS. If interested, you can review our Pupil Privacy Rights Policy #5500 on the SVSU website at www.svsu.org.
Should you decide to opt your child out of participating in the survey please inform us by requesting a paper copy of this letter, which has an opt-out form at the bottom. Contact Clinical Coordinator, Stacey Subik, at 447-7511 x2442 for details.
